A 54 YEARS OLD MALE PATIENT WITH TUBERKULOSIS AND ARTHRITIS GOUT

Abstract


Tuberculosis (TB) was an infectious disease of lung caused by Mycobacterium tuberculosa found in many developing countries such as Indonesia and usually occurs in children and adults. Reported cases of pulmonary tuberculosis accompanied with gout arthritis in male patients aged 54 years with complaints of cough since ± 6 months ago, sweat at night, and drastically reduced weight. From the results obtained pulmonary physical examination: rhonki + / +, wheezing - / -, there was a swelling of the lower extremities in the region of the first metatarsophalangeal joint. Neurological status: normal physiological reflexes, pathological reflexes (-). Results of xray examination showed diffuse tuberculosis, results of sputum smear was positive  (+ / + / +), and the results of uric acid levels was 12.4 mg / dl. Therapy given to patients were isoniazid 300 mg, rifampicin 450 mg, ethambutol 1000 mg and 1000 mg pyrazinamide. Allupurinol dose of 3 x 300 mg per day was used as the treatment of chronic gout and 90% effective in reducing uric acid levels. It can be concluded that the treatment of tuberculosis with and without gout arthritis same are same. It also requires a long time to prevent recurrence, complications and transmissions. It involves the role of psychosocial problems and participation of health workers and family members to do interventions for patient.
